About OHCP OHCP is an organization
whose goal is to improve the health of Douglas and Sarpy Counties through a
community-driven process. This is accomplished by partnering with
neighborhoods to work together to identify community assets, prioritize the
issues, and develop solutions that rely on the assets of the community.

In 2002, a community health assessment was conducted which examined reported
health behaviors and perceptions of quality of life. The survey identified
nine key community health improvement areas:
Cancer, Diabetes, Heart Disease and Stroke, Infant Health, Lung Disease,
Sexually Transmitted Diseases, Mental Health and Substance Abuse, Violence
and Abusive Behavior, and Activities Designed for Youth.
OHCP then created a measurement system to mark
progress toward its vision. The measurement system views the community’s
health through the use of indicators – a numerical measure of quality of
life. A broad range of Stakeholders developed the indicators for the nine
key areas. These indicators will be monitored and reported annually.
The indicators serve as a springboard for community discussions to identify
methods that are successful and to implement long-term change strategies.
They also serve as a tool for education and strategic thinking. As the
indicators were developed, the OHCP discovered something important –
everything is linked to everything else. Douglas County is a great place to
live with a wealth of resources. However, our most valuable resource lies
within us – our community. This report examines the current data and
outlines a vision for the future.
What do these indicators mean to me?
This is an exciting opportunity to begin community-wide surveillance of the
nine key community health improvement areas. Throughout this process, OHCP
will act as the catalyst and convener. We are just one piece of the puzzle,
however. It will take the efforts of each partner to realize the OHCP
vision. It is our hope that our entire community will adopt these indicators
for both personal and strategic action. We ask individuals and organizations
in the community to consider the following questions:

impacted by myself and my organization?

How do the indicators relate to our
strategic mission and vision?

What is my commitment as a leader to
positively affect the trends shown by the indicators?
